CMC Terms and Conditions
CMC Terms and Conditions required for any participation with Community Music Connection (herein also referred to as "CMC"):
Publicity Release I give my permission to Community Music Connection and its officers and directors - for no compensation - to use my or my child’s or my spouse's (or any other person's) image, likeness, voice, live appearance, or performance (including any form of photo, audio, video, or other recording) in promotions, advertising, public relations, or other programs, including live, print, and digital formats. Liability Waiver I understand that Community Music Connection and its officers, directors, agents, employees, volunteers, facility providers, and contractors assume no responsibility for any injuries or illness that I or my child or any other person may sustain while observing or participating in CMC activities or while using any facilities or equipment during any CMC activities. I understand CMC and its officers, directors, agents, employees, volunteers, facility providers, and contractors shall not be responsible for any property lost, damaged, or stolen before, during, or after CMC activities. I, for myself, my child, my spouse, my heirs, and any other person associated with my or their involvement with CMC, do hereby release and indemnify CMC and its officers, directors, agents, employees, volunteers, facility providers, and contractors from any and all claims for injury, illness, loss, or damage I or my child or any other person may suffer as a result of any participation with CMC, including any injury, illness, loss, or damage caused by negligence, if any, of CMC, its officers, directors, agents, employees, volunteers, facility providers, or contractors, or the negligence of anyone else. COVID-19 Protocols I understand and agree that properly designed masks worn properly to fit snugly against the sides of the face and completely covering the nose and mouth will be required of each participant and observer while indoors-unless such requirement is explicitly waived by the CMC Executive Director. By order of the governor of the State of Texas, "no entity in Texas can compel receipt of a COVID-19 vaccine by any individual, including an employee or a consumer, who objects to such vaccination for any reason of personal conscience, based on a religious belief, or for medical reasons, including prior recovery from COVID-19." However, Community Music Connection strongly recommends that each participant and observer be fully vaccinated and boosted against COVID-19. I understand and agree that (unless explicitly allowed by the CMC Executive Director) persons in the following circumstances will not be permitted to participate in, observe, or be indoors with any CMC activities: persons who are experiencing or exhibiting any COVID-19 related symptoms, persons who are aware or believe that they have been exposed to a person or animal with a positive and confirmed case of COVID-19 within the past 14 days, persons who have tested positive for COVID-19 within the past 14 days. I understand and agree that the interpretation and decision of the CMC Executive Director will be final with respect to all COVID-19 Protocols. Cancellation Policy CMC will not issue refunds if any CMC activity is missed or canceled due to adverse weather, health concerns, or other reasons. Agreement In consideration for participation with CMC, on behalf of myself, my child, my spouse, my heirs, and any other person associated with my or their participation or membership in Community Music Connection, this registration with CMC is my digital signature and indicates and confirms my understanding and agreement to all of the CMC terms and conditions, publicity release, liability waiver, COVID-19 protocols, and cancellation policy. |
